What to ask your insurance provider when you call about coverage:
Do I have coverage for outpatient mental or behavioral health?
Do I have a co pay?
Do I have co insurance?
Do I have an individual deductible?
Do I have a family deductible?
Does the individual or family deductible have to be met for mental or behavioral health?
If I went to an appointment today, what would my out-of-pocket amount be?
Is pre-authorization required?
If so, how do I get pre-authorization?
Is there a pre-authorization number I need to give to the counselor?
Are behavioral diagnoses covered under this policy?
Am I up to date on my premium?
If you are seeking couples counseling, ask if couple's counseling is covered? If yes, ask if it is covered under couple's counseling or family counseling.
If you are seeking an ADHD assessment, ask if this is covered by your policy? Ask if you need to see a certain type of professional (such as a PhD)
Filing your insurance
We will file insurance on your behalf if we are in-network with your insurance carrier. If you have primary insurance and secondary insurance, we may be in-network with one, but not the other. If we are in-network with your primary insurance but not your secondary insurance, we will file the primary insurance on your behalf and you will be responsible for filing the secondary insurance. You will be responsible for your co pay, co insurance, and/or deductible at the time of your appointment.
If we are in-network with your secondary insurance, but out-of-network with your primary, you will be responsible for the full payment at the time of service. We will be happy to file your secondary insurance if you bring us your EOB from filing your primary insurance. In the event that we receive reimbursement from your secondary insurance, we will reimburse you the amount we received from the secondary insurance. We have no way to file secondary insurance without an EOB from your primary insurance. We are under time constraints to file insurance. If you would like us to file your secondary insurance, please turn in the EOB to us as soon as possible.
